Maari Ma Health seeks a Connecting Care Worker to support Aboriginal and Torres Strait Islander people, especially older and vulnerable community members, to access culturally protected health, aged care, disability, housing, social and community services.
You will work with clients, families, carers, health professionals and community organisations to provide advocacy, care coordination, referral support, health system navigation and practical assistance to improve health, wellbeing and independence.
