Keep Connected Support Coordination is expanding and looking for a passionate and hard-working person to join our team! This is a strictly work from home role however occasional travel to meet participants and stakeholders may be required. Keep Connected is a small independent provider located in South East Melbourne. We are a registered NDIS provider catering to all NDIS participants for Support Coordination and Specialist Support Coordination (Level 2 and 3).
Qualifications & experience
Tertiary education in either Disability, Community Services, Social Work, Psychology or substantial industry experience
NDIS Worker Screening Clearance
Current Drivers License
Working with Children’s Check
Previous experience working as a support coordinator or in a case management role
Solid knowledge of the disability and community services sector
Experience working with people with disabilities, child protection involvement, homelessness, Alcohol and Other Drugs and mental health
NDIS Online Orientation Module
Tasks & responsibilities
Assist clients to exercise choice and control by understanding and implementing their NDIS plans
Support clients to connect to NDIS and mainstream services
Support participants through planning meetings and reviews
Adhere to thorough case note recording and prepare NDIS reports as required in line with support coordination requirements
Meet weekly KPI targets set out by Keep Connected
Work autonomously with solid organizational skills and self motivation
