Divine Support Care is seeking compassionate and dedicated Disability Support Workers to deliver high-quality, person-centred support to individuals living with disability. The successful candidate will assist participants to achieve their goals, enhance their independence, and actively participate in their communities while maintaining their dignity, choice and control.

Key Responsibilities

· Provide person-centred support in accordance with individual support plans and participant goals.

· Assist participants with personal care, mobility support and daily living activities.

· Support participants with household tasks, meal preparation and maintaining a protected home environment.

· Facilitate community access, social participation, recreational activities and appointments.

· Encourage the development of life skills, independence and confidence in daily routines.

· Assist participants with travel and transport arrangements where required.

· Provide emotional support and foster positive, respectful relationships with participants and their support networks.

· Administer or assist with medication in accordance with training, policies and participant requirements.





· Maintain accurate documentation, progress notes and incident reports.

· Work collaboratively with families, guardians, allied health professionals and the wider support team to achieve positive participant outcomes.

Qualification and Skills

· Certificate III or Certificate IV in Individual Support (Disability), Disability, Community Services, Aged Care or a related qualification.

· Previous experience supporting people with disability, complex needs or community-based care services will be highly regarded.

· Demonstrated understanding of person-centred care principles and participant rights.

· Solid interpersonal, communication and relationship-building skills.

· Ability to work independently, use initiative and adapt to varying participant needs.

· Understanding of behaviour support, risk management and safe work practices.

Salary: AUD $75,000 – $85,000 plus superannuation

Employment Type: Full time and 38 hours per week

Pay: $75,000.00 – $85,000.00 per year
