The Eyre and Far North Local Health Network (EFNLHN) is seeking a Care Partner - Support at Home (Mid-West) for the Elliston Community & Allied Health team. This permanent part time 0.5 FTE role offers EN or ASO3 pathways, with appointment based on experience and skills, working to support older people at home under CHSP programs.
