Job Description
n
Build and maintain referral pathways and shared care arrangements with general practitioners, practice managers and primary care stakeholders
n
Develop and sustain relationships with general practices, primary care networks and community partners
n
Promote the Mental Health Multidisciplinary Team program and act as a key liaison between WMQ and external stakeholders
n
Facilitate communication and collaboration across multidisciplinary and primary care teams
n
Assist with care coordination activities and smooth transitions between the program and participants' broader care networks
n
Identify opportunities to strengthen stakeholder engagement, service accessibility and referral outcomes
n
Represent WMQ within the community and engage stakeholders in their own settings
n
n
Requirements
n
n
Relevant tertiary qualification in Public Health, Community Development, Social Work, Human Services, or a related discipline, or equivalent experience
n
Experience building stakeholder relationships and referral pathways within primary care, healthcare, or community services
n
Strong community engagement, partnership development, or advocacy experience
n
Ability to deliver presentations and communicate effectively with a range of audiences
n
Understanding of and commitment to working with diverse communities, including Aboriginal and Torres Strait Islander peoples, CALD, Pasifika and LGBTQIA+ communities
n
Proof of COVID-19 vaccination status,
or as required by legislation and WMQ policy
n
Relevant probity checks required by legislation and WMQ Limited policy
n
Current Driver's Licence and willingness to travel for work
n
