This full time role offers the chance to bring your clinical knowledge to a professional, supportive team and play a meaningful role in delivering protected, effective, and person-centred care. No two days are the same! As Clinical Care Coordinator you will:
n
n
Coordinate the admission of new residents, completing comprehensive assessments and developing person-centred care plans in collaboration with healthcare professionals.
n
Lead, mentor, and support the care team to deliver safe, high-quality clinical care and positive resident outcomes.
n
Provide hands-on nursing care, including medication administration, wound management, and ongoing clinical monitoring.
n
Maintain accurate clinical documentation, oversee infection control practices, and ensure compliance with regulatory standards and audits.
n
Communicate effectively with residents, families, and external healthcare providers while driving quality improvement initiatives.
n

About You
n
n
Bachelor of Nursing with current AHPRA registration.
n
Minimum 2 years' nursing experience in residential aged care, with experience in a similar clinical care role highly regarded.
n
Demonstrated ability to work collaboratively within multidisciplinary care teams.
n
Strong clinical assessment, documentation, and evidence-based practice skills.
n
Excellent communication and interpersonal skills to engage effectively with residents, families, and colleagues.
n
Proficient computer and information management skills, including Microsoft applications and clinical software systems.
n
Sound understanding of the aged care accreditation process and ability to meet Aged Care Worker Screening legislative requirements.
