The Connecting Care Worker plays a vital role in supporting Aboriginal and Torres Strait Islander people, particularly older people and vulnerable community members, to access culturally safe health, aged care, disability, housing, social, and community support services.
Working closely with clients, families, carers, health professionals, and community organisations, the Connecting Care Worker provides advocacy, care coordination, referral support, health system navigation, and practical assistance to ensure individuals receive the services they need to improve their health, wellbeing, independence, and quality of life.
The role promotes a holistic and culturally responsive approach to care that aligns with the values, traditions, and strengths of Aboriginal and Torres Strait Islander communities.
The priority client target group for this position is Aboriginal people aged over 55 years who are identified as frail and/or vulnerable. Aboriginal people aged over 55 years who are functioning independently are not a priority for access to the services provided by the Connecting Care Worker.
This position works in Wilcannia and may deliver outreach services to other towns in our region.
Salary up to $*****gross per hourplus super
Maari Ma offers a great range of benefits including:
Working Monday to Friday 8.30am to 5.00pm

All Maari Ma Health employees are required to:
have a cleared NSW Working with Children Check. (We will support you to obtain if you don't have one)
undertake a National Police Check. Having a criminal record will not necessarily stop you from being employed. We will talk with you about it privately.
Comply with the Maari Ma Staff Immunisation Policy. This is a category A position, Screening & vaccination for diphtheria, tetanus, pertussis, hepatitis B, measles, mumps, rubella, tuberculosis, varicella, and annual Influenza is mandatory
