Right at Home Noosa, Gympie, Hinterland is expanding our Support at Home team, seeking a Clinical Care Partner to bring heart, skill and leadership to every interaction while providing clinical leadership, care coordination and oversight for clients.
You will manage your own caseload, conduct clinical assessments, develop care plans and support care staff to ensure services align with client goals and budgets, delivering person-centred care in clients' homes.
