Job Description
The Aboriginal Mental Health Care Navigator at Sydney Local Health District will ensure Aboriginal and Torres Strait Islander consumers access holistic, culturally protected mental health services across care settings.

You will work to develop and strengthen connections between Community Mental Health, non-government organisations, primary care, social support, justice and drug and alcohol services to reduce duplication and improve transitions in care for Aboriginal people.
