Job Description
n
Provide assessment, brief intervention, case management and coordinated care to adults with complex mental health needs
n
Conduct comprehensive mental health assessments and develop individualised, recovery-oriented treatment plans
n
Deliver evidence-based psychological and psychosocial interventions
n
Provide case management and care coordination across participants' care journeys
n
Partner with GPs and healthcare providers to deliver shared care, clinical consultation and coordinated support
n
Facilitate referrals to appropriate health, community and support services
n
Collaborate with a multidisciplinary team to provide integrated, person-centred, trauma-informed and culturally safe care
n
Maintain accurate clinical records, manage risk and safety planning, and ensure compliance with clinical governance and quality standards
n
n
Requirements
n
n
Qualified in Social Work, Psychology, Occupational Therapy, Counselling, Mental Health Nursing or a related discipline
n
Experience supporting people with complex mental health needs through assessment, intervention and case management
n
Skills in risk assessment, safety planning and evidence-based, recovery-focused practice
n
Comfortable working independently and as part of a multidisciplinary team
n
Ability to build relationships with clients, families, GPs and community partners
n
Strong communication, time management and clinical documentation skills
n
Commitment to person-centred, trauma-informed and culturally protected care
n




Proof of COVID-19 vaccination status, or as required by legislation and WMQ policy
n
Relevant probity checks required by legislation and WMQ Limited policy
n
Current driver's licence and willingness to travel for work
