About Yallambee – Millmerran.
n
Located in Millmerran, Yallambee is a 60 bed residential care home, providing general residential care as well as dedicated dementia support.
n
Yallambee incorporates several accommodation areas –
n

Kooroongarra Lodge – Secure dementia care accommodation
n
Yandilla Rooms – additional dementia specific accommodation
n
Bringalily Lodge – broader residential accommodation
n

The Opportunity
n
We're looking for an experienced and motivated Care Manager to join our team and lead the delivery of high-quality, person-centred care at Yallambee, McLean Care.
n
This is a full-time, onsite leadership role where your clinical expertise, compassion and ability to inspire others will directly influence the wellbeing and quality of life of our residents.
n
You'll be a trusted leader on the floor — working alongside residents, families and care teams to ensure care is secure, responsive and genuinely tailored to each individual.
n
You'll mentor and empower your team, champion best practice and help create an setting where residents receive exceptional care and staff feel supported to do their best work.
n

As Care Manager, you will:
n
n
Lead and contribute to a collaborative team to ensure high-quality, person-centred care is delivered to residents and their representatives.
n
Mentor, support and guide staff, promoting excellence in clinical care, lifestyle services and team capability.
n
Ensure duty of care is upheld in line with organisational policies, professional codes, ethical standards and legislative requirements.
n
Monitor, evaluate and report on clinical practices, ensuring alignment with best practice, governance and compliance requirements.
n
Participate in audits, reviews,



data analysis and continuous improvement initiatives to enhance resident outcomes.
n
Oversee care planning in collaboration with residents, families and clinical teams.
n
Support staff development through training, performance reviews, mentoring and education initiatives.
n
Build strong relationships with internal teams, external providers and community stakeholders.
n
Remain responsive to operational requirements, including after-hours availability when required.
n
n
About You
n
You're a confident and compassionate clinical leader who knows that great aged care starts with great people.
n
You'll bring:
n
n
A Bachelor Degree in Nursing (Registered Nurse)
n
Current registration with AHPRA.
n
Minimum 3 years' experience in residential aged care.
n
Strong knowledge of the Aged Care Quality Standards, Aged Care Act principles, documentation standards, residents' rights, funding instruments and relevant legislation.
n
Well-developed clinical knowledge and contemporary nursing skills.
n
Excellent communication and interpersonal skills.
n
The ability to build trust and engage effectively with residents, families and staff.
n
A current National Police Certificate, or willingness and ability to obtain one.
n
Previous nursing leadership and/or accreditation experience is desirable.
n
n
