Job Description
Country Health Connect is seeking a Home Support Worker to assist older people and others in their own homes within the South East region. You will work under a Consumer Directed Care model, delivering practical, person-centred help as outlined in each consumer's care plan.

duties include housekeeping, laundry, meal preparation, and support services. You will work independently in the community while collaborating with health professionals to ensure protected, respectful and high-quality care.
