Job Description
UnitingCare Queensland is seeking a Supported Living Facilitator to provide on-the-go in-home care and daily living assistance. You will drive between clients, support independence, and help with activities that improve quality of life. Must have a positive attitude and willingness to learn.

Role includes driving to clients, planning activities, and working within Care Plans in a team-focused workplace. Emphasis on safety, inclusivity, and client-centred care across Queensland communities.
