Role Summary As an in-home nurse, you will deliver person-centred clinical care to adults and children with diverse disability-related needs.
You will work across the Campbelltown area, providing assessments, medication management, wound care, health education, and liaison with allied health and primary care providers.
This role is ideal for a motivated clinician who enjoys autonomy, values teamwork, and seeks professional growth.
Key Responsibilities Provide comprehensive nursing care in clients' homes, prioritising dignity and choice.
Conduct clinical assessments, risk assessments, and regular care plan reviews.
Administer and manage medication safely, including documentation and reconciliation.
Deliver wound care, continence support, catheter and PEG tube management as required.
Implement behaviour support strategies in collaboration with behaviour specialists.
Educate clients, families, and support workers on health conditions and care techniques.
Maintain accurate,
timely clinical records in accordance with organisational and legislative standards.
Coordinate with GPs, allied health professionals, and other stakeholders to optimise outcomes.
Participate in team meetings, clinical supervision, and continuous professional development.
Essential Requirements Qualification: Bachelors degree in Nursing.
Registration: Current AHPRA registration as a Registered Nurse.
Licences Checks: Full Australian Drivers Licence, police check, and Working With Children Check.
Proven experience in community or disability nursing, aged care, or related clinical settings.
Strong communication, time management, and clinical judgement skills.
Ability to work independently and as part of a multi-disciplinary team.
