Disability Support Worker Durack, NT, Full Time, Healthcare Medical Vacancy Listing Dates 14/09/2026 to 12/10/2026 Please Quote Reference Number ***** Skilled Disability Support Worker required Full time opportunity Join the team at Territory True Pty Ltd We are seeking a compassionate, reliable and motivated Disability Support Worker to join Territory True Pty Ltd.
The ideal candidate is committed to providing person-centred support tailored to each participant's individual needs, routines and goals, while promoting their choice, independence and wellbeing.
This role requires a hands-on approach to assisting participants with personal care, daily living activities, household tasks, community access and social participation.
The successful candidate will demonstrate strong communication and interpersonal abilities, empathy and patience, and the ability to work both independently and collaboratively while maintaining a safe, respectful and supportive environment for participants.
Your role will include, but is not limited to: Provide person-centred support based on each participant's individual needs, routines and goals.
Assist participants with personal care, including hygiene, dressing, grooming and toileting.
Assist participants with mobility and support during daily activities, where required.
Support participants with daily living and household tasks.
Assist with shopping, menu planning, meal preparation, cooking, laundry and cleaning.
Support participants to develop and maintain independence and daily living skills.




Organise and facilitate social activities, outings and community participation.
Accompany and transport participants to appointments, activities and social engagements.
Provide companionship, friendship and emotional support.
Support participants to build confidence, skills and social connections.
Provide appropriate supervision to maintain participant safety and wellbeing.
Communicate with allied health professionals and implement recommended support strategies in accordance with participant care plans.
Implement participant health plans and individual support strategies as directed.
Complete daily progress notes and document relevant observations, activities and participant updates.
Maintain safe, clean and presentable participant living environments.
Follow organisational, NDIS, health and safety policies and procedures.
Applicants must have: Relevant trade qualification AQF Certificate II or III (ANZSCO Skill Level 4) or at least one year of relevant experience.
Robust verbal communication and interpersonal abilities.
Empathetic, patient and respectful approach to participant support.
Ability to promote participant choice, control and independence.
Confidence in supporting community access, social participation and engagement.
Sound knowledge and understanding of disability and its impact across different life stages.
A salary of $70,000 - $85,000 will be offered to the successful candidate.
