YMCA Victoria in Horsham is seeking a part time NDIS Support Navigator & Coordinator to assist people with disabilities in accessing services and living independently.
You will implement NDIS plans, connect with providers, and support goal achievement in a person-centred manner.
Ideal candidates have 12+ months in disability support, knowledge of NDIS legislation, and a Certificate IV in Community Services or equivalent; WWCC and a valid driver's license are required.
