What You'll Do:
n
In this dynamic and rewarding role, you'll:
n
n
Partner with participants to design and implementpersonalised, strengths-based NDIS plans.
n
Connect people with the services and supports that alignwith their goals and needs.
n
Use recovery-oriented and person-centred approaches tobuild skills and confidence.
n
Coordinate service delivery, monitor progress, and adaptplans as required.
n
Collaborate with a multi-disciplinary team and contributeto service innovation.
n
Maintain accurate records and ensure alignment with NDISpractice standards.
n

What You'll Bring:
n
n
Significant experience in the disability or human servicessector.
n
Knowledge of intervention models and ability to coordinateservices.
n
Specialist knowledge of NDIS and related supportservices.
n
Strong problem-solving and conflict-resolution skills.
n
Ability to advocate effectively for participants andpromote their independence.
n
n
Your Toolkit:
n
n
Tertiary qualification in community services, social workor equivalent and extensive community work and training(essential)
n
Current DHS Working with Children Check (essential)
n
Current SA driver's licence (essential)
n
Current Safe Environments for Children and Young People –Through Their Eyes Certificate (essential)
n
Support Coordination experience (preferred)
n
n
