Where you'll be working
Join the Rehabilitation in the Home (RITH) team, a multidisciplinary team with rehab staff specialist leadership who see clients recently discharged home from hospital in the Newcastle, Lake Macquarie and Port Stephens LGAs. RITH is a 6 week home based rehab program for under 65 yrs.
Based in Newcastle, you will work closely with physiotherapists, social workers, dietitians and rehabilitation medical staff to deliver person-centred, goal-focused care.

The Opportunity
In this role, you will:
? Provide high-quality specialist occupational therapy service to patients and their families/carers with Rehabilitation in the Home (RITH)
? Assess patients in their home environment and support their return to meaningful daily activities.
? Provide recommendations for equipment, home modifications and strategies to improve independence and safety.
?Collaborate with patients, families and carers to develop and achieve rehabilitation goals.
?Work as part of a supportive multidisciplinary team focused on improving patient outcomes and reducing hospital readmissions.
?Empower patients to build confidence, manage their health and maximise their independence.

This position is line managed by the Allied Health Service Manager for RITH and professional support is provided by the Head of Discipline for Occupational Therapy in Integrated Care, Network and Partnerships. Clinical support is provided by other members of the MDT as well as the other Occupational Therapist's working alongside you in other programs. Orientation will be provided to ensure a smooth transition into the role. The Occupational Therapist team schedule regular supervision times to encourage up to date evidence-based practice. You will report to the Allied Health Service Manager
