Central Coast Local Health District is seeking a Community Nurse for the Virtual Care Team. You will combine clinical nursing with virtual care to support patients transitioning from hospital to home, promoting independence and preventing unnecessary readmissions.

The role requires RN registration with AHPRA, a passion for patient-centred care, and robust communication. Travel across the Central Coast and use of virtual care tech are involved.
