Remuneration: $61.31 - $63.29 + Superannuation + Salary Packaging

Where you'll be working

Tomaree Community Hospital is a well-resourced rural facility providing emergency, aged care, and outpatient services to the Port Stephens community. Working within a close-knit, well-established multidisciplinary team, the successful applicant will deliver high-level clinical social work support across acute and sub-acute settings. Key responsibilities include complex case management and discharge planning, clinical consultation, and service enhancement.

This position sits within the Lower Hunter Social Work Team, operation and professional reporting will be to the Head of Discipline for Social Work Lower Hunter. Onsite support will be provided by the Tomaree Community Hospital Health Service Manager.

The Opportunity

This Social Work position works across all ward within Tomaree including Medica, and Emergency Department. Social work receives referrals through the electronic medical record. The focus of the role is providing psychosocial interventions to patients in Tomaree hospital. Social work will undertake psychosocial assessment, conduct family meetings complete applications for Guardianship and provide case management of complex patients. The role is supported by the Lower Hunter Head of Social Work. The Line manager of the position is the HOD of Social Work.



Monthly supervision is an essential requirement for this role. The position sits in the Lower Hunter Social Work Department who met monthly. Ongoing professional development is encourage and supported through the Lower Hunter Allied Health Department. The position requires a social worker who can work independently without direct supervision.

Requirements

- Must hold as a minimum a bachelor degree in social work which provides eligibility for membership of the Australian Association of Social Workers, or other qualification deemed equivalent by the employer.
- Current driver's license and willingness to travel
