Marathon Health is seeking a Care Coordinator in Wagga Wagga, NSW (Wiradjuri Country), to bridge gaps to local health services, improve health literacy, and build capacity in self-management. This part time role (30 hours/week) focuses on personalised care coordination for individuals experiencing homelessness within the community.

You will engage with Aboriginal and CALD communities, assess needs, develop care plans, and advocate for clients within the healthcare system.
