Calvary Health Care is seeking a Clinical Project Lead for the Dementia Model of Care to lead implementation across hospital, aged care and home care services. You will work with consumers, families and clinicians to ensure consistent, person-centred dementia care and lead education and capability-building initiatives.

The role partners with service leaders to assess readiness, address barriers, and drive sustainable practice change, reporting on progress and outcomes across Calvary services.
