The Role An opportunity has become available for a Clinical Nurse Consultant (Transition Care Team) to join Gold Coast Health (GCH) where the focus of our 14 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
Offer advice and attend joint home visits to manage complex clients
Manage a small team of clinical nurses in their day to day clinical practice with clients
Manage a clinical workload, including bi-weekly allocation of clients and delegation of clinical duties
Actively participate and drive high level quality service and professional development
About you To be successful in this position you will require:
Bachelor of Nursing or equivalent
Registration with the Nursing Midwifery Board of Australia
Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
Tuberculosis screening, to identify if the tuberculosis vaccination may be required
Declaration of serious discipline history must be disclosed
Criminal History Screening will be required for new employees (GCH will cover cost)

Base salary ranging from $140,632 - $152,685 per annum
About Us The Transition Care Program aims to provide a comprehensive multidisciplinary service to eligible clients over the age of 65 (55+ for Aboriginal and Torres Strait Islander peoples)
to assist with the transition from hospital to home, prevent unnecessary admissions to residential care, and reduce hospital length of stay. The TCP nursing discipline's primary clinical purpose is to assess, support and educate clients with nursing needs and provide evidence based clinical care across a broad spectrum of chronic and acute conditions.
