Clinical Nurse Specialist – Palliative Care
Permanent Part-Time | 32 hours per fortnight (0.4 EFT)

Benalla Health is seeking a compassionate and experienced Clinical Nurse Specialist (CNS) – Palliative Care to join our dedicated team.

About the role
As the Palliative Care Clinical Nurse Specialist, you will provide specialist nursing support across the palliative care journey, working closely with clients, carers and families to ensure care is responsive to their individual needs, goals and preferences.
You will
- Coordinate the intake and prioritisation of palliative care referrals.
- Undertake comprehensive specialist assessments and develop goal-oriented, person-centred care plans in partnership with clients and their carers/families.
- Regularly review and evaluate client progress, adapting care as needs, goals and preferences change.
- Provide specialist advice, education and support to clients and carers managing life-threatening illness.
- Act as a key worker and consistent point of contact for clients and families regarding their care.
- Work collaboratively within a multidisciplinary team to coordinate responsive and seamless care.
- Build strong relationships with GPs, specialists, medical consultancy services, district nursing, allied health,
community services, volunteers and loss and grief support.
- Promote client-led decision making, health behaviour change, chronic disease self-management and the palliative approach.
- Advocate for clients and families, ensuring their wishes, values and preferences remain central to care.
About you
We are looking for an experienced nurse who combines strong clinical expertise with empathy, confidence and a genuine commitment to supporting people and families at a significant time in their lives.
You will bring
- Postgraduate experience in Palliative Care Nursing.
- Community nursing experience.
- Demonstrated skills in working with consumers and families with a life-threatening illness and an understanding of the palliative approach.
- Demonstrated knowledge and skills in Health Behaviour Change, Chronic Disease Self-Management, consumer-led Goal Directed Care Planning, and Person-Centred Care.
- Current Car driving licence
