About us:
Cairns and Hinterland Hospital and Health Service (CHHHS) provide a level 5 Cardiology Service as classified by the Clinical Services Capabilities Framework. The service delivers interventional and non- interventional inpatient and outpatient services for patients located in both the CHHHS and Torres and Cape Hospital and Health Service (TCHHS).

About your opportunity:
The Health Worker - Advanced is required to provide culturally safe and effective cardiac health service delivery and program management that focuses on health promotion, prevention and early intervention of health issues in Aboriginal and Torres Strait Islander communities. The position provides care under the direct or indirect supervision of a Registered Nurse or Clinical Nurse Consultant, which supports the delivery of an efficient and effective cardiac health programs to contribute to better health outcomes for Aboriginal and Torres Strait Islander people transitioning between the hospital and community setting.
About you:
In your new role you will....
Delivercore primary health care programs such as cardiac care and/or chronic disease education, prevention, detection and management and screening activities according to the needs of the Aboriginal and Torres Strait Islander communities in a confidential and non-judgemental manner using culturally acceptable strategies.
Liaise with community members and other health professionals to advise, support and encourage community participation and involvement in the promotion and delivery of cardiac health care, and ensure that the health needs of the Aboriginal and Torres Strait Islander communities are being met.
Establish and maintain a communication network between health service providers and the communities by attending meetings, liaising with councils, Aboriginal Medical services and other providers.
Maintain appropriate and accurate documentation by using established guidelines and programs.
