Santa Clara Home Care is an approved Support at Home provider. We are looking for a Registered Nurse who is compassionate, organised and confident working independently, to coordinate care for older people living in their own homes across Sydney.

About the role

- Coordinate and advocate: care planning, in-home assessments, client and family consultations, stakeholder communication, and clinical guidance.
- Support clients and frontline staff: roster oversight, care worker onboarding, training compliance, urgent shift coverage, and quality monitoring.
- Keep care safe: risk assessments, clinical incident reporting, documentation, collaboration with families and health professionals, and identifying hazards in the home.

Remuneration Above-award hourly pay under the Nurses Award 2020, based on your applicable classification and recognised nursing experience, plus a negotiated premium. Superannuation, and any applicable penalties, overtime and allowances, are paid in addition. Travel time between client visits is paid and kilometres are reimbursed.

Hours

Permanent part time, with paid leave entitlements. Your days and hours are negotiated with you, and we will agree your guaranteed minimum hours and rostering arrangements in writing before you start, so you know exactly what you are committing to.

Preferably 2 full days either on Tuesday, Wednesday or Thursdays from 8am - 4pm or as negotiated and agreed

About you

- Current AHPRA registration as a Registered Nurse.
- At least 1 year of community, home care or aged care nursing experience.
- A national police certificate issued within the last 3 years, or a valid NDIS Worker Screening Clearance.




- Current First Aid and CPR certification.
- COVID-19 vaccination in line with our workforce policy.
- A full NSW driver licence, a registered and insured vehicle, and a smartphone.
- Strong communication and clinical documentation skills, and the confidence to make sound decisions in a client’s home without a supervisor down the hall.
