How You'll Make an Impact
- Partner with clients, families, and key stakeholders to deliver personalised care that supports independence, wellbeing, and choice.
- Manage a portfolio of Home Care clients, ensuring care plans remain current, outcomes-focused, and aligned with individual needs, goals, and funding.
- Conduct regular client assessments and reviews to identify changing needs and coordinate appropriate supports.
- Build strong relationships with clients and care teams to ensure high-quality, person-centred services are delivered with compassion and excellence.
Our Ideal Candidate
- Qualified in Individual Support, Ageing Support, Case Management, or a related field.
- Experienced in care coordination, client assessments, and leading care teams.
- Knowledgeable in CHSP and Support at Home programs.
- Confident using Microsoft Office and digital care management systems.
- An excellent communicator with strong organisational and problem-solving skills.
- Holds a current Driver Licence, NDIS Worker Screening Check, and First Aid Certificate.
- Passionate about delivering exceptional care and supporting older people to live independently.
