About the opportunity
Are you an experienced Care Manager or Community Care Manager looking for an opportunity to have a genuine impact on the way community care is delivered? Nurse Next Door Kiama is looking for an experienced Registered Nurse to take ownership of clinical quality, care delivery, team leadership and the client experience across our growing community care service.
This is a senior leadership position for someone who already understands the realities of community care and is confident managing complex client needs, leading frontline teams, working with families and professionals, and balancing clinical quality with the operational demands of a growing service.
Based from our Dapto office, you'll also spend time in the community visiting clients, meeting families, supporting our team and building strong relationships with referral partners across the Illawarra.

What you'll be responsible for
As our Care Manager / Community Care Manager, you'll have genuine ownership of the clinical, operational and people leadership of our community care services.
Your responsibilities will include
- Leading, mentoring and developing our Caregivers and Care Ambassadors.
- Providing clinical leadership and escalation support to the care team.
- Overseeing complex care, including catheter management, wound care and comprehensive clinical assessments.
- Assessing new clients and conducting Caring Consults with clients and families.
- Designing and reviewing personalised care plans around each client's goals, needs, risks and funding.
- Monitoring client care and responding appropriately as needs change.
- Managing clinical incidents, risks, complaints and quality issues.
- Maintaining high standards of clinical documentation, compliance and care delivery.
- Working closely with Care Services and Scheduling to ensure the right people are delivering the right care at the right time.
- Building strong relationships with GPs, hospitals, allied health professionals, families, support coordinators and other referral partners.
- Supporting recruitment, onboarding, coaching and performance development of our care team.
- Using AlayaCare and other digital systems to maintain strong visibility across care, communication and documentation.
- Identifying opportunities to improve systems, processes, clinical standards and the overall client experience.
About you
This is an experienced management position. We're looking for someone who has already worked in a Care Manager, Community Care Manager, Clinical Care Manager, Case Manager or similar senior community-care leadership role.
You'll bring
- Current AHPRA registration as a Registered Nurse.
- Demonstrated experience managing community-based care services.
- Proven experience leading, mentoring and supporting frontline care teams.
- Strong clinical knowledge and confidence managing complex care.
- Hands-on experience with catheter management and complex clinical assessments.
- Experience developing, reviewing and overseeing individualised care plans.
- Strong knowledge of Aged Care, Support at Home and/or NDIS.
- Experience managing clinical incidents, risks, complaints, documentation and compliance.
- Excellent communication and relationship-building skills.
- Strong organisation and the ability to manage competing priorities.
- Confidence using digital care-management systems, including AlayaCare or similar.
- A practical, solutions-focused approach and a willingness to find a better way.
- A current Australian driver's licence.
Checks & requirements
You'll need, or be willing to obtain, the relevant employment screening checks, including:
- Aged Care National Police Check
- Working With Children Check
- NDIS Worker Screening Check
What we offer
- $115,000–$125,000 + Super, depending on experience.
- Monday to Friday working arrangements.
