Care Partner – Support at Home | Part-Time

Alpha Care And Share | Regents Park / Logan / Brisbane South

Permanent Part-Time | Approx. 18 hours per week across 3 days

$45–$50 per hour + superannuation + mileage reimbursement

Alpha Care And Share is expanding into the Support at Home aged care program and is seeking an experienced and motivated Care Partner to help establish and grow our aged care services across Logan and Brisbane South.

This is an excellent opportunity for an experienced Care Partner, Care Manager, Case Manager or Home Care Coordinator who enjoys supporting older people and their families while also building strong professional relationships within the local community.

The role will initially be approximately 18 hours per week across 3 days, with the opportunity for hours to increase as our Support at Home client base grows.

About the Role

As our Care Partner, you will support clients from initial enquiry and onboarding through to assessment, care planning, service coordination, monitoring and ongoing review.

During the early growth phase of our Support at Home service, you will also play an important role in developing referral relationships with local:

- GP clinics
- Hospitals
- Retirement communities
- Allied health providers
- Pharmacies
- Community organisations
- Other aged care and healthcare professionals

As our client base grows, the role will progressively develop into a larger Care Partner / Care Management portfolio.

Key Responsibilities

Client Intake, Assessment & Care Planning

- Respond to enquiries from prospective clients and families
- Support clients through intake and onboarding
- Conduct person-centred client assessments
- Identify client needs, preferences, goals and risks
- Develop, implement and regularly review individualised care plans
- Apply wellness, reablement and independence principles
- Respond appropriately to changes in client health, function or circumstances

Care Coordination

- Coordinate personal care, domestic assistance, social support, transport, nursing, allied health and other approved services
- Liaise with clients, families, support workers, Registered Nurses, GPs, hospitals, allied health professionals and external providers
- Conduct regular client reviews and welfare checks
- Monitor service quality and client outcomes
- Identify and escalate clinical or safeguarding concerns appropriately
- Advocate for client choice, dignity and independence

Budget & Service Monitoring

- Monitor client service utilisation and Support at Home budgets
- Help clients and families understand how available funding is being used
- Ensure services align with care plans and available funding
- Identify under-utilisation or over-utilisation of funding
- Support appropriate use of third-party providers
- Assist with Assistive Technology and Home Modifications where applicable




- Work with management and administration to support accurate service delivery and billing

Documentation & Compliance

- Maintain accurate and timely assessments, care plans, progress notes and client records
- Record care management activities and client communications
- Work in accordance with the Aged Care Act, strengthened Aged Care Quality Standards, Support at Home requirements and organisational policies
- Participate in quality improvement and service development activities

Community Engagement & Referral Relationships A key part of this role, particularly during our launch and growth phase, will be helping Alpha Care And Share build a strong presence within the local aged care community.

You will

- Visit GP clinics, hospitals, retirement communities and allied health providers
- Build professional relationships with local referral partners
- Follow up referral opportunities professionally and consistently
- Attend relevant networking and community events
- Introduce Alpha Care And Share’s Support at Home services to potential referral partners
- Respond promptly and professionally to referral enquiries
- Help build awareness of Alpha Care And Share across Logan and Brisbane South

This is not a commission-based sales role. We are looking for someone who can build genuine professional relationships while maintaining a strong focus on quality care and ethical aged care practice.

About You

We would particularly like to hear from applicants with previous experience as a:

- Care Partner
- Care Manager
- Case Manager
- Care Coordinator
- Home Care Coordinator
- Community Aged Care Coordinator

You will ideally have:
- Experience in Support at Home, Home Care Packages or community aged care
- Experience conducting client assessments, care plans and ongoing reviews
- Understanding of My Aged Care and the Support at Home program
- Experience monitoring client services and budgets
- Knowledge of person-centred care, wellness and reablement
- Strong communication and relationship-building skills
- Confidence communicating with clients, families, healthcare professionals and referral partners
- Confidence undertaking community visits and developing referral relationships
- Strong organisational, documentation and time-management skills
- Ability to work independently and manage competing priorities
- Valuable computer literacy and confidence using care management systems

Qualifications A relevant qualification in Aged Care, Individual Support, Community Services,



Nursing, Allied Health, Health Services, Case Management or a related field will be highly regarded. A Registered Nurse or Enrolled Nurse background is highly regarded but not essential.

Strong practical experience in community aged care, Home Care Packages or Support at Home will be highly valued.

Essential Requirements

- Current Australian driver’s licence
- Access to a reliable vehicle
- Current First Aid and CPR, or willingness to obtain
- Ability to meet applicable aged care worker screening and police check requirements
- Current unrestricted working rights in Australia

Local Experience We are particularly interested in applicants who understand the Logan, Brisbane South and surrounding aged care community.

Existing qualified relationships with local GPs, hospitals, retirement communities, allied health providers or other aged care referral sources will be highly regarded.
