This position requires availability to work Mondays and Fridays, with the candidate demonstrating a willingness to work extra weekend and afternoon shifts when shifts are available.

About The Chance

Join our Community Nursing team and provide high-quality, person-centred nursing care to clients across the Central Coast. Working as part of a multidisciplinary team, you will support clients to maintain their health, independence and wellbeing within the community. In this role you will:

- Provide comprehensive nursing assessments, care planning, case management and clinical care to clients in the community.
- Promote health, wellbeing and independence through education, support and health promotion activities for clients and carers.
- Work collaboratively with clients, carers, health professionals and external service providers to coordinate effective care and achieve positive outcomes.
- Apply evidence-based nursing practice and contribute to quality improvement and the ongoing development of community nursing services.
- Manage a varied workload both autonomously and as part of a multidisciplinary team, maintaining high standards of professional and responsive care.

About You

You are a compassionate and adaptable Registered Nurse who is committed to delivering high-quality, person-centred care in the community. You will bring strong communication skills, sound clinical judgement and the ability to work effectively both independently and as part of a multidisciplinary team. We are looking for someone who has:

- Current registration as a Registered Nurse (Division 1)



with AHPRA and a current Australian Driver Licence.
- Recent experience in Community Nursing, or a demonstrated interest in developing your skills and experience in community-based care.
- Strong communication, interpersonal and negotiation skills, with the ability to build positive relationships with clients, carers and health professionals.
- Excellent time management and organisational skills, with the ability to prioritise a changing workload and work autonomously.
- A commitment to evidence-based practice, quality improvement and delivering safe, responsive and person-centred care.
