Company Description The Filipino Community Council of Victoria Inc. (FCCVI) is the umbrella organisation representing Filipino-Australian communities across Victoria. It is a duly incorporated, non-political, non-stock, and not-for-profit benevolent community service organisation.
FCCVI is dedicated to supporting the wellbeing, cultural identity, and social participation of Filipino-Australians. The organisation works collaboratively with community groups and service providers to deliver programs that respond to the needs of older adults, families, and individuals. Role Description The CHSP Lifestyle Coordinator is responsible for planning, coordinating, and delivering lifestyle and social support programs under the Commonwealth Home Support Programme (CHSP) for older community members. This full-time, in office role based in Melbourne, VIC involves designing activity schedules, organising group and individual engagement programs, and ensuring activities promote independence, wellbeing, and social connection.
Day-to-day tasks include assessing client needs, liaising with families and caregivers, maintaining accurate records and documentation, and monitoring program outcomes in line with CHSP guidelines. The Coordinator collaborates with staff, volunteers,
and external partners to ensure services are culturally appropriate and accessible. The role also requires ongoing evaluation of programs, risk management, and adherence to quality and safety standards.
Qualifications
- Demonstrated experience in community services, aged care, or lifestyle coordination, ideally within a CHSP or similar program context.
- Strong skills in program planning, activity design, and client engagement, with a focus on promoting independence and social inclusion.
- Excellent communication and interpersonal skills, including the ability to work effectively with older adults, families, volunteers, and multidisciplinary teams.
- Sound organisational and time-management abilities, including maintaining accurate documentation and meeting reporting and compliance requirements.
- Understanding of person-centred care, culturally responsive practice, and relevant aged care and CHSP standards.
- Proficiency with basic computer applications and data entry for scheduling, record-keeping, and reporting.
- Relevant qualification in community services, aged care, social work, or a related discipline (or equivalent experience).
- Current police check, Working with Children Check (if required), and eligibility to work in Australia; First Aid certification is an advantage.
- Knowledge of or connection to Filipino culture and communities in Victoria is desirable but not mandatory.
