- Base Salary = $93,516 + Superannuation with Salary Packaging up to $15.9k

About The Opportunity

As our phone-based Client Service Navigator – Aged Care, you will provide specialist service navigation and end-to-end coordination for aged care funding including Support at Home and Commonwealth Home Support Program (CHSP). In this role you will also:

- Take full ownership of the client journey from referral through to service commencement, acting as the single point of contact for Support at Home stakeholders
- Provide expert advice to Support at Home Program Managers and Case Managers on how Vision Australia equipment and services can be funded for Support at Home clients
- Build and maintain proactive, effective relationships with Support at Home Program Managers and Case Managers

About You





As part of the National Contact Centre, you should have demonstrated experience in client service, intake, service navigation or coordination roles. In addition, a strong understanding of aged care funding models, including Support at Home, CHSP (My Aged Care) is encouraged. You will also have:

- Ability to interpret and apply funding rules to service delivery scenarios
- Strong organisational and coordination skills with ability to manage multiple tasks
- Excellent communication and stakeholder engagement skills
