Join our current program supporting young people across the Darling Downs, Sunshine Coast and Gold Coast. You’ll be part of a service that works closely with local Hospital and Health Services to deliver an integrated model of care, combining clinical insight with psychosocial support.
As a Complex Case Worker, you’ll provide specialist psychosocial support to young people experiencing early psychosis. You will lead complex case management, coordinate tailored care plans, support positive recovery outcomes, and work collaboratively with families, mental health services, and community partners. You will also provide guidance to team members and help deliver integrated, person-centred care for young people across the region.
Responsibilities
- Provide advanced case management and support for young people experiencing early psychosis, including complex and high-risk presentations.
- Deliver recovery-focused psychosocial support that promotes wellbeing, independence, and positive outcomes for young people aged 12-25.
- Support leadership teams with service coordination, complex referrals, and team guidance.
- Facilitate individual and group programs for young people, families, and carers, including psychoeducation and wellbeing support.
- Build strong partnerships with health and community services to ensure coordinated, culturally safe,
and seamless care pathways.

Requirements
- Tertiary qualifications in allied health, social or community services, or equivalent demonstrated industry experience
- Experience providing psychosocial support in a mental health setting, preferably with young people and families.
- Current Driver Licence
- Current Blue Card
- Current National Police Check
