About the Role

This role supports GPs providing medical services to aged care facilities across Adelaide. You will work closely with treating doctors, residential aged care facility (RACF) staff, and other healthcare providers, alongside the Agedcare GP team, to deliver high-quality resident care.

Responsibilities include conducting comprehensive medical assessments, maintaining accurate electronic medical records, facilitating effective communication between stakeholders, and travelling between assigned facilities to provide timely and efficient support.

Your time will be divided between various RACFs across Greater Adelaide and a shared working space.

Subject to team leader approval, some CMA documentation may be completed offsite.

Key Responsibilities

- Conduct Comprehensive Medical Assessments (CMAs) for residents in aged care facilities.

- Collaborate with treating doctors to ensure high-quality, resident-centred care.

- Facilitate the onboarding of new doctors to facilities and support doctors transferring between facilities.

- Maintain accurate and up-to-date electronic medical records.





- Travel between assigned facilities within your designated hub to deliver timely and efficient services.

- Attend Medication Advisory Committee (MAC) meetings with pharmacies and facility representatives to support safe and effective medication management.

- Coordinate resident vaccination programs, including liaising with pharmacies regarding consent and vaccine supply.

- Ensure all Medicare and MyMedicare requirements are met each quarter.

- Meet with treating doctors at least monthly to discuss resident care, clinical priorities, and facility needs.

- Support treating doctors with care initiatives, including Residential Medication Management Reviews (RMMRs), overdue CMAs, care plan reviews, case conferences, and family meetings.

About you

- AHPRA nurse qualification

- Ability to travel as necessary to RACFs across Greater Adelaide and to office/shared working space locations
