Job Description

Monash Health is seeking a Care Partner for the Transition Care Program in the Dandenong region. This part time role (0.6 EFT, 45.6 hours per fortnight, 3 days/week) focuses on supporting older people to transition from hospital to home or ongoing care with person-centred, coordinated support.

You will work across Home-Based and Bed-Based TCP services, building relationships with clients and families, and collaborating with geriatricians and the broader team to achieve goals and secure outcomes.
