The role links consumers with high intensity AOD support to suitable community services and works with complex cases to ensure a seamless transition with explicit goals.

Part time (0.8 EFT / 64 hours) until January 2027, based at St Kilda Road Clinic, you will join an experienced multidisciplinary team and contribute to coordinated care for clients with complex needs.
