Sky-High is a division of Jack's Butterflies where we take our ethos of providing holistic and unwavering support and advocacy to any person living with a disability and require support to live their best life.
We are a team filled to the brim of skilled professionals, clinical nurses specialising in paediatrics and adults with acute care requirements, physiotherapists who tailor programs for land, water and respiratory physio, specialised support coordinators and individuals with extensive real lived experience of the sector.
We have recently expanded into the aged-care sector and all realms of disability care and are looking for compassionate, reliable and dedicated people who share our ethos to join our team.
ABOUT THE ROLE...
Support Coordinators form part of a highly skilled team of allied health professionals in supporting children, young people, adults and their caregivers to live a fulfilled life by assisting them to achieve their NDIS goals and explore mainstream services.
We have a waitlist of clients and are looking to expand our team with support coordinators already active in this sector.
Level 2 and Level 3 Support Coordinators are responsible for assisting clients and their families in achieving their goals and maximising their potential.
You will work with participants to ensure the quality and appropriateness of their supports, and to assist in addressing life challenges that may rise (unwellness, changes in circumstances, safety and risks).

This role will be based in our Chermside office and will require travel for home visits when needed.
Responsibilities...
Manage a clinical caseload, including assessment, planning, and evaluation of NDIS plans.
Assist clients in implementing their NDIS plans, engaging high-quality service providers.
Coordinate complex care requirements, ensuring optimal support utilisation.
Report and participate in NDIS client review processes.
Maintain a caseload of 6.5 billable hours per workday.
Assisting the pro-bono team supporting clients with limited/no funding to cover their needs.
Encourage client and family participation in program evaluation.
Engage support networks to resolve conflicts and build participant capacity.
Evaluate interventions to meet goals and integrate with other Sky-High services.
Liaise with carers, families, and service providers to enhance quality of life.
Contribute to NDIS service planning and reporting as required.
About You...
Knowledge of best practice interventions, Strength-Based, and Child & Family-Centred practices.
Excellent written communication skills, including high-quality report preparation.
Experience working in multi-disciplinary teams to deliver individualised interventions for children with complex needs.
Proven ability to maintain professional networks to promote best practices for people with disabilities.
Strong interpersonal and collaboration skills, with the ability to work with people with disabilities, their families, and support networks.
Ability to work autonomously and collaboratively in a team setting.
Understanding of person-centred thinking, NDIS principles, and National Disability Services Standards.
You will have a vast knowledge of main stream services and NDIS funded supports.
