Job Description
Launch Housing is seeking a part-time Case Manager for the Supporting Families Program, based in Collingwood, Melbourne.
You will deliver outreach-based housing support to families with children, develop and implement tailored case plans, and link clients to education, training and health services.
This maximum-term role runs until end of March ****, 68.4 hours per fortnight, with a solid focus on child-centred practice and collaboration with local services.
