Job Description
South Western Sydney Local Health District is seeking a Registered Nurse for the Transitional Aged Care Program in Camden.
The role focuses on delivering holistic nursing care, assessments and education within a community/home setting, supporting reablement and independence.
You will perform nursing assessments, wound care, and managediabetes/medicationeducation, while coordinating with a multidisciplinary team to ensure protected hospital-to-home transitions.
