Southern NSW Local Health District is seeking a Senior Aboriginal Health Worker for the Aboriginal Chronic Care Program based at Mount Druitt Hospital.
You will support Aboriginal and Torres Strait Islander people to access culturally secure chronic care services and act as a trusted link between communities, healthcare providers and support services.
You will advocate for culturally safe practices, develop referral pathways, deliver education and health promotion, and collaborate across hospital,
