Western Health invites applications for Gem at Home, a junior doctor role within our community-based care team in Melbourne.
You will work with an Advanced Trainee and a Geriatrician who provide mentorship as you develop skills for General Practice or Geriatric Medicine.
The role focuses on enabling earlier discharge for patients over 65, delivering geriatric assessments at home, and coordinating care with hospital networks.
Full time or part-time options are negotiable.
