Across the state, we are dedicated to offering exceptional care, accommodation, and support services for older Western Australians.About The RoleJuniper is seeking experienced Registered Nurses to join our Home and Community Care team supporting clients across the Perth metro area.
In this role, you will provide clinical care and nursing support to clients living independently in the community.
You will conduct assessments, develop and review care plans, and work collaboratively with clients, families and healthcare professionals to ensure safe, high-quality care at home.
This is a rewarding opportunity for a compassionate and motivated nurse who is passionate about supporting older Australians to maintain their independence and wellbeing in the community.
Juniper is currently offering a casual contract; the successful candidate must be available Wednesdays and 1 weekend day a week.
As this is a community-based role, travel between client homes is required.




Applicants must hold a current driver's licence and have access to a reliable vehicle.Key ResponsibilitiesConduct clinical assessments and respond to changes in client health and wellbeingDevelop, implement and review person-centred care plans with clients and their familiesProvide clinical care including medication management, wound care and health monitoringSupport clients to maintain independence and wellbeing in their own homesLiaise with GPs, allied health professionals and other service providers to coordinate careProvide clinical advice and support to community care staffMaintain accurate and timely clinical documentation and reportingEnsure compliance with organisational policies and Aged Care Quality StandardsParticipate in quality improvement and secure clinical practice initiativesKey RequirementsCurrent AHPRA registration as a Registered NurseExperience in aged care, community care or a similar clinical settingStrong clinical assessment and care coordination skillsAbility to work independently in a community-based roleExcellent communication, organisation and problem-solving skillsCurrent driver's licence and willingness to travel between client homesCurrent National Police Clearance (or willingness to obtain)What's On OfferAttractive remuneration dependent on experienceSalary packaging up to $18,550 per annum tax free17.5% annual leave loadingAccess to Juniperks, our employee benefits platform offering retail discounts, wellbeing initiatives and recognition programs
