About the role:

Centacare is looking for a Clinical Care Partner to deliver person-centered care, comprehensive clinical assessments, and ongoing care coordination to help older Australians live safely and independently in their own homes.

- Employment Status: Full Time – Monday to Friday hours from 8:30am – 5pm.
- Location: Working primarily across Darra (South Brisbane) with some travel to clients’ homes with mileage reimbursed.
- Remuneration: $58ph + Super + Not for Profit Salary Packaging (Increases your take home pay) + Allowance Travel Reimbursement (KMu0027s).

Key Responsibilities:

- Complete clinical assessments, care planning, reviews and care coordination.
- Monitor client health and wellbeing, escalating concerns and maximise independence.
- Work collaboratively with Care Partners, Clinical Manager, Allied Health Professionals, GPs, and specialists to ensure clients’ holistic care.
- Provide clinical guidance and support to Care Partners and Support Workers.
- Ensure compliance with aged care standards and support clinical governance.
- Collaboration and support clinical-related investigations, including Serious Incident Response Scheme (SIRS) requirements.
- Work with clients and families to sustainably manage care budgets.

Skills and Experience:

- Experience in care planning, case management, and community aged care or a similar clinical environment.
- Strong understanding of person-centred care, clinical governance and multidisciplinary practice.
- Strong clinical decision-making skills and a genuine commitment to supporting older people to maintain their independence, dignity and quality of life.
- Experience with Restorative Care, End-of-Life pathways is highly regarded.
- Experience with mentoring or providing clinical guidance to team members will be highly regarded.

Essential Requirements:

- Bachelor of Nursing
- Current AHPRA registration as a Registered Nurse
- Current driver’s licence
- Reliable, registered and insured vehicle
- Current paid Blue Card
- Current paid NDIS Worker Screening Clearance
- Current First Aid and CPR certification
- Australian Federal Police Criminal History Check
- Evidence of the right to work in Australia
