Right at Home Noosa, Gympie, Hinterland is expanding its Support at Home team and seeks a Clinical Care Partner to provide clinical leadership, care coordination and oversight for clients receiving services through the Support at Home Program.
You will manage your own caseload, conduct clinical assessments, develop care plans, support care staff and ensure services align with each client's goals and budget while maintaining high standards of safety and person-centred care.
