Team Leader – Transition Care Program (Home Based)

n

- Part time ongoing chance | 64 hours per fortnight

- Based at Royal Park Campus

About the role

n

Transition Care Program – Home Based is a jointly funded program (State/Commonwealth) and aims to assist frail aged patients to complete their restorative processes at home, optimise their functional capacity and finalise their long‐term care arrangements.

n

The Team leader will take a leading role within the Transition Care Program – Home Based, including quality planning, policy review, budget management, aged care compliance and ensuring the services are aligned with organisational objectives and professional standards.

n

The role is responsible for identifying and resolving client and staff issues, while overseeing service delivery to ensure a high‐quality experience for clients. Working closely with the Program Clinical Lead, you'll support effective client service provision, coordination and flow across the program.

n

Key Accountabilities

n

n
- Work closely with a highly experienced multidisciplinary team to support the delivery of case management to older people transitioning from hospital to home.
n
- Work collaboratively with internal and external stakeholders to build strong relationships and partnerships, strengthen service models and improve patient flow.
n
- Provide leadership and support to the team, contributing to quality service delivery, continuous improvement and effective resolution of client and staff issues.
n
- Contribute to the ongoing development of the team and service,



supporting initiatives that enhance care for older people and strengthen Community Services across RMH.
n
- Support quality planning, compliance and resource management, ensuring the program aligns with relevant aged care requirements, organisational objectives and professional standards.
n

About You

n

You'll bring a strong understanding of aged care, community services and hospital processes, along with the ability to support older people with complex care and social needs. You'll also bring:

n

n
- Registration as a Division 1 Registered Nurse or relevant experience as an Allied Health Professional.
n
- A strong understanding of aged care services and legislative requirements, including My Aged Care.
n
- Experience working with clients, families, support networks and service providers to monitor and evaluate service delivery and care needs.
n
- Demonstrated skills in comprehensive assessment and discharge planning.
n
- Sound knowledge of hospital processes, aged care and community service systems.
n
- Experience supporting older people, including those with complex social circumstances and care needs.
n
- Experience working across hospital, aged care and/or community settings.
n
- Previous experience leading and supervising teams, with the ability to support staff and promote effective service delivery.
n
- A flexible, proactive and innovative approach, with the ability to work effectively both autonomously and collaboratively within a multidisciplinary team.
