About the Role
Our client is a registered organization dedicated to community development and support services. We are seeking a dedicated Community Worker to join their team and deliver impactful programs across local communities.
Key Duties and Responsibilities
- Conduct in-depth client assessments to identify needs, then design and implement tailored educational, training, and support programs that address social, emotional, and financial challenges.
- Interview clients to evaluate the nature and extent of their difficulties, and develop individualized case plans with clear, measurable goals.
- Monitor client progress through regular check-ins, document outcomes, and prepare detailed reports for internal review and external agencies.
- Refer clients to appropriate services—such as housing, employment, or health providers—and coordinate multi-agency support to ensure holistic care.
- Assess community needs and resources related to welfare, housing, employment, training, and other facilities, then advocate for improved services.
- Liaise with community groups, welfare agencies, government bodies,
and private businesses to address local issues and promote awareness of available resources.
- Support families and care for children, disabled persons, or elderly clients in group housing, adult service units, and institutional settings.
- Supervise offenders on probation or parole, ensuring compliance with court orders and facilitating rehabilitation programs.
- Assist young people in resolving social, emotional, and financial difficulties through counselling and practical guidance.
- Prepare funding submissions and detailed reports for government bodies and other agencies to secure resources for community initiatives.
Requirements
- Minimum 1 year full time relevant experience (disability support, community services, or allied health assistance).
- Relevant qualification (e.g., Certificate IV/Diploma in Community Services, Disability, or Mental Health).
- Strong interpersonal and communication skills for effective liaison with diverse stakeholders.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Current driver’s license and willingness to travel within the community.
