- Empower people to achieve their goals, improve mobility and maintain independence through person-centred care
- Deliver rehabilitation, exercise and recovery programs
- Work across Private Services, Rehab in the Home (RITH), Hospital in the Home (HITH) and Support at Home programs
- Deliver coordinated care alongside Clinical Care Partners, nurses and allied health professionals
- Support prevention, early intervention and reablement for older Australians
- Contribute to the future growth and development of the Support at Home service
- Work as part of a cooperative Allied Health team
Requirements
- Current unrestricted AHPRA registration as a Physiotherapist
- Current driver's licence and access to your own vehicle
- Passion for helping people improve their mobility, confidence and quality of life
- Comfortable working autonomously in the community while being part of a supportive team
- Excellent time management and organisational skills
- Vaccination status declaration and/or evidence may be requested depending on the role

Demonstrates expertise in delivering person-centred rehabilitation and recovery programs while collaborating effectively with clinical care partners and allied health professionals. Holds current AHPRA registration as a Physiotherapist and possesses strong organizational and time management skills.
