HammondCare is seeking a permanent part time Clinical Partner - Nursing to support clients living at home in the Central West/Nepean/Blue Mountains region. This role is three days per week, Monday to Friday, within the Home Care Clinical Care & Practice Team to assess needs, set goals and deliver tailored, person-centred care in clients’ homes.

You will work with an experienced multidisciplinary team to provide care, perform home visits or group activities, and contribute to continuous quality
