Job Description

St Vincent's Health Australia Limited in Melbourne is seeking a registered nurse to deliver Hospital in the Home (HITH) care, conducting comprehensive patient assessments, developing individualized care plans, and delivering IV therapies and other advanced nursing interventions.

You will monitor patients with acute and complex needs, coordinate referrals and discharge planning, work with a multidisciplinary team, mentor junior staff, and contribute to quality improvement and service development
