Job Description

n
- Conduct foundational assessments and ensure quality documentation of resident needs
n
- Develop individual care plans
n
- Manage and refer AN-ACC reviews through My Aged Care to ensure resident funding
n
- Apply the Australian National Aged Care Classification (AN-ACC) to achieve financial KPIs while balancing care needs and resident choice
n
- Communicate with clinical and care colleagues, management and residents to ensure quality documentation outcomes
n
- Apply comprehensive and accurate nursing assessments and nursing practices
n
- Educate, motivate and guide colleagues
n
- Coordinate work and establish priorities to meet deadlines and commitments
n
- Travel to other communities and work within a broader team
n

Requirements

n

n
- Registered Nurse qualification with current AHPRA registration
n
- Demonstrated knowledge of AN-ACC, including assessments, claims submissions and working within policies and guidelines
n
- Proven knowledge of current trends in nursing and current clinical expertise in health and aged care
n
- Understanding of new Aged Care Quality Standards and the relationship with AN-ACC
n
- Demonstrated previous experience with funding business rules and assessment procedures, including AN-ACC validations
n
- Highly developed interpersonal communication and negotiation skills, particularly in relationships with older people, general practitioners and other healthcare providers
n
- Ability to coordinate work, establish priorities, and meet deadlines and commitments
n




- Knowledge of Autumn Care highly regarded
n
- Experience in commissioning an aged care facility advantageous
n
- Valid national police check
n
- Successful completion of a pre-employment medical assessment
n
- Must work onsite in Beachmere, Queensland
n
- Must be willing to travel to other communities
n
